I LINDA CARYLL now of Number 35 Brentford Road Cross Roads Post Office in the Parish of Saint Andrew in the Island of Jamaica, Widow, DO HEREBY SINCERELY AND SOLEMNLY DECLARE:-

1. I was formerly Linda Taperell of Great Britain and I was married in the year one thousand nine hundred and Seven in this island to Caryl Abrahams of Hayfield in the Parish of Saint Catherine in this island, Plantar. By Deed Poll dated the Twenty third day of April One thousand nine hundred and Twenty one and duly recorded in the Office of the Deputy Keeper of the Records of this Island, my said husband changed his name to Caryl George Blackwin Caryll and since that date I have been known by the name of Linda Caryll.

2. My husband the said Caryl George Blackwin Caryll (formerly Abrahams) died on the Second day of April One thousand nine hundred and Thirty-five. I am the mother and surviving parent of Mary Agnes Brian Caryll who was born on the Fifth day of May One thousand nine hundred and Eighteen at Hayfield in the Parish of Saint Catherine aforesaid and I exhibit: herewith marked with the letter "A" a certified copy of the register of the birth of said Mary Agnes Brian Caryll. My said daughter is now training to become a Nurse at Bedford County Hospital in England AND I MAKE THIS SOLEMN DECLARATION conscientiously believing the same to be true and by virtue of the provisions of an Act made and passed in the Sixth year of the reign of Her Majesty the late Queen Victoria entitled an Act to abolish oaths and affidavits except in certain cases and to substitute declarations in lieu thereof and to suppress voluntary and extra-judicial oaths and affidavits.
